JURY TRIAL SCHEDULE

PLEASE READ BELOW FOR INSTRUCTION IF YOU HAVE BEEN SUMMONED
JURY EXEMPTION AND QUALIFICATION RESPONSE REQUIREMENT
If you have received a Jury Summons and questionnaire, you are required to submit a response (even if the trial has been cancelled) showing that you are eligible and qualified to serve or you must submit a claim for exemption.
You may complete and submit your questionnaire when you report for jury duty on the day of the trial listed above and on your notice.
If you are requesting an EXEMPTION, you must complete the information on the questionnaire and submit the juror questionnaire form with any additional information. If you are requesting a RESET, you must complete and submit the juror questionnaire form with a written request to be reset.
Exemptions or Reset Requests can be submitted to the court by mail, in person or by email to Municipal Court Public Inbox http://Municipal Court Public Inbox <firstname.lastname@example.org> and must be received by the court at least one (1) week prior to your jury service date listed above. IF YOU ARE EMAILING YOUR RESPONSES TO THE COURT, DOCUMENTS MUST BE IN THE FORM OF A PDF ATTACHMENT. NO PHOTO COPIES OF DOCUMENTS WILL BE EXCEPTED. Your answers are CONFIDENTIAL and may be disclosed only to the Judge, court personnel, the litigant and the litigant’s attorney.

If you do not submit a response to your jury summons or request a reset within the time referenced above, you may be required to appear for a contempt hearing before the Municipal Court Judge.
